Literature Review of the Effect of Tongue-pressure Resistance Training Using the IOPI on Tongue Strength and Swallowing Function in Patients with Dysphagia

Abstract

|
This article is a review of literature published during the last decade regarding the effect of tongue-pressure resistance training(TPRT) using the Iowa Oral Performance Instrument (IOPI) on tongue strength and swallowing function in patients with dysphagia. The training protocol consists of exercises performed at a strength of 80% of the maximum tongue pressure, which has differed slightly across studies. We identified that the application of TPRT using the IOPI showed significant improvement in tongue strength and swallowing function in dysphagia patients with brain damage of various etiologies. Some studies did not demonstrate a significant difference in the improvement of tongue strength and swallowing function compared with conventional dysphagia rehabilitation treatment. However, TPRT using the IOPI, which is focused on the improvement of tongue pressure, can help with the rapid and safe movement of a bolus from the mouth to the pharynx by increasing intraoral pressure, and its positive effects on decreasing residues and aspiration have also been identified. In conclusion, TPRT using the IOPI may be an effective intervention for improving tongue strength and swallowing function in patients with dysphagia, but if more systematic research is supported, it can be clearly presented as a evidence-based exercise.
